Birth and Death Records
The Mesa County Health Department can issue
certified copies of records for births from 1903 to the present for all Colorado counties.
Certified copies of death certificates are available for deaths which occurred in
Mesa County.
How to Obtain Records
Apply in person for same-day service or mail in an application with your check or
money order. A copy of your picture ID is required when requesting birth or death certificates. Certified birth and death records are also available on the Web with credit card
payment at VitalChek.

Fee Schedule for Colorado Vital Records
There is a fee of $17.75 for the first copy or for a search of the files when no record
is found; additional copies of the same record, obtained at the same time, are $10.00 each.
About Birth and Death Records
Local registrars work under the direction and supervision of the state registrar and
are subject to the Vital Statistics Act [C.R.S. 25-2-103-25-2-105] and Regulations
[Section 2.3 and 2.4]. Those responsible shall promptly file an accurate certificate with
the local registrar in the county of birth or death.
The local registrar maintains copies of birth, death, and fetal death certificates
[more than twenty weeks gestation], prepares reports and performs other duties as
required by the Office of the State Registrar. In on-line counties with electronic birth
certificate reporting, the on-line electronic record is the local registrar's copy.
All Colorado vital records are confidential in accordance with Colorado statutes
[C.R.S. 25-2-117]. |
