Birth and Death Records
Local registrars work under the direction and supervision of the state registrar and are subject to the Vital Statistics Act(C.R.S.25-2-103-25-2-105) and Regulations (Section 2.3 and 2.4) All Colorado birth and death records are confidential in accordance with Colorado statutes (Section 23-2-117).
Birth records are available for all counties in Colorado. There is a fee of $17.75 for the first copy or for a search of the files when no records are found; additional copies of the same record, obtained at the same time, are $10.00 each.
Death records are available for deaths which occurred in Mesa County only. There is a fee of $17.00 for the first copy or for a search of the files when no records are found; additional copies of the same record, obtained at the same time, are $10.00 each.
Policy Issuing Certified Birth and Death Records
All vital statistics offices will review applications for certified copies of birth or death certificates to determine the applicant’s direct and tangible interest. The official will request supporting documentation when the applicant’s direct and tangible interest in the requested document is unclear. This supporting documentation will be used to establish proof of relationship and/or proof of a direct and tangible interest. A photocopy of the documentation establishing relationship and /or a direct and tangible interest must be attached to each application. Current driver’s license is also required.

How to obtain Records
Apply in person for same day service or mail an application with your check or money order. A copy of your picture ID is required.Mesa County Health Department does not accept credit cards or online orders; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc.
