The Community Health Needs Assessment (CHNA)

We release a Community Health Needs Assessment (CHNA) every three years in partnership with non-profit hospitals in the County.
The Community Health Needs Assessment identifies opportunities to build community-driven solutions that make our residents, businesses, schools, and neighborhoods more resilient.
This approach is rooted in the belief that community-driven solutions should be a collaboration between community partners and across sectors to creatively address our most challenging areas of action.

Other Special Reports
- Health Care Access in Mesa County– In 2015, 40% of emergency department visits in Mesa County could have been treated by a routine provider. This report helps explain why. The report includes information on barriers to care, Medicaid and CHP+ enrollment, and ratios of primary care physicians, physician assistants, nurses, psychologists and dentists.
