Grand Valley Connects offers a centralized system to access and navigate potential services and resources, including mental health.  Our goal is to assist community members with identifying eligibility, support the enrollment process, and help navigate barriers that community members may experience.  We make the enrollment process easier.

Grand Valley Connects is committed to being a partner and leader in an effort to improve access to services through simplified processes and reliable coordinated care for individuals and community partners.

We know that access to services can be complicated. We also know that accessing those services could mean the difference between achieving financial independence, economic stability, or worrying about putting food on the table for local families.

Our supportive team approach helps to increase resource awareness, normalize access to needed services, and empower members in their pursuit of health and financial stability for themselves and their families.

We’re All About Transformation

Transformation through community-led, cooperative approach to Mesa County behavioral health and economic stability by supporting sustainable systems of care, prevention, and inclusion.   

As part of our Behavioral Health System Improvement Plan, Grand Valley Connects will be a single point of entry for Mesa County residents and their caregivers to access comprehensive behavioral health information and assistance with system navigation to ensure best outcomes for our residents.
