If you did not save or cannot find your confirmation for the Mesa County Public Health COVID-19 vaccine interest list, you can look up your submission below.
Please understand this form is NOT an appointment for vaccine. We use this interest list to contact individuals and provide them with scheduling information.
After filling out the form please watch for an email from Mesa County Public Health (MCPH) for scheduling.
We suggest checking your junk or spam folders (since we send this as a mass notification to numerous individuals).
Questions About Your Status?
My interest list status says, “Contact Attempted” but I wasn’t contacted by Mesa County Public Health (MCPH).
The communication will come from MCPH in an email. In addition, we are working with our local hospitals so it’s possible you were offered scheduling at one of their clinics. If your status says contact attempted and you have not received an email link for scheduling from anyone, we recommend checking all spam and junk folders.
We ask that after you submit the form you regularly check the email address you shared with us. If after checking all folders you do not believe you ever received an email link, please send an email or call 970-248-6900 and press option 7 for vaccine information, then option 2 for scheduling. Please note, we are not able to resend this information, it is a unique code sent once to individuals.
What about individuals who do not have email or asked to be contacted via phone?
We are working through the list of individuals who have called and left contact information for us to call back and make scheduling arrangements. This is manual process so we are not able to provide an online search function at this time.
How do I confirm my vaccination appointment?
This status inquiry is for our interest list, it is NOT an appointment for vaccination.
Once the status above changes to contact attempted, you will receive an email from Mesa County Public Health with instructions on how to register for your vaccine appointment. This can be done through our self-scheduling tool.
Through the link you are sent, you can make, or adjust your appointment if needed, and can also print or email a copy of your vaccine appointment confirmation for your records or to bring with you to your vaccination appointment.
Were you vaccinated elsewhere?
If you filled out the Vaccine Interest form from Mesa County Public Health but were vaccinated elsewhere and would like to be removed from our list, please unsubscribe here. This will remove you from the Mesa County Public Health contact list for vaccination scheduling and we will no longer email you scheduling or appointment updates.
